Request a Replacement Certificate

If you’ve lost your Segment 1 or Segment 2 certificate of completion, we can help you request a replacement. Please follow the instructions below carefully:

Step 1: Email Your Information

Send an email to capitolareadriving@gmail.com with the subject line:
"Lost Certificate"

​

In your email, include the following details:

• Student’s full name (first, middle, last)

• Student’s birthdate

• Current mailing address

• Email address

• Which certificate you need: Segment 1 or Segment 2

• The month and year you took the class

Step 2: Send Payment

Please send a money order for $10 (no checks or cash, please) to:
